What to tell us when scheduling
- That your concern is related to a workplace injury.
- The date of injury and the body part involved.
- Your employer’s name and workers’ compensation insurer, if known.
- The claim number and Workers’ Compensation Board case number, if available.
- Where you were previously evaluated and whether you already have imaging or treatment records.

Care built around function
Your provider will review the injury, examination findings, previous care, and the physical demands of your work. Bring a description of your job duties, any current restrictions, relevant records, and the questions you want to address.
The treatment discussion may include nonsurgical care, rehabilitation, imaging, injections, or surgery when appropriate. Work recommendations and follow-up plans depend on your individual evaluation.
